Our Rapid Hospital Discharge Service supports participants who are ready to leave hospital to return home or find medium and long term housing, and coordinate the care and support services they need.

Our experienced team will:

Manage the process of transitioning you back into the community, in close collaboration with you and your support network.

Coordinate with our Allied Health partners to ensure your home or supported living environment is safe and ready for you.

Connect you with suitable Medium Term Accommodation (MTA) options for a safe discharge and support further planning to transition you to a long-term home.

Work with our Supported Disability Accommodation provider partners to find the perfect accommodation that suits your needs.

Provide highly trained support workers to deliver your 24/7 Supported Independent Living supports.

Develop a person-centered support plan and Roster of Care that suits your individual needs and goals.

Ensure your goals are proactively supported within your plan.

Partner with nursing and allied health professionals to deliver your supports.

Advocate for your needs & coordinate change of funding submissions if your circumstances change.

Oversee your clinical care to reduce risk of hospital re-admission.
